HIPAA Privacy Policies
As part of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), healthcare practitioners must now follow a set of federal regulations that address your rights to privacy and handling of your Protected Health Information (PHI). Your privacy is important to us at Bare Compounding Pharmacy. Linked below you will find documents describing how your personal health information is protected. When you come into the pharmacy to pick up a prescription, you will be asked to sign an acknowledging receipt of the Bare Compounding Pharmacy Notice of Privacy Practices. If you choose to have your prescription mailed, please print out the Acknowledgement of Receipt of Notice of Privacy Policies linked below, fill out and return to us.
